The service and support is for patients, family & community members who self-identify with Aboriginal Ancestry - First Nations (on & off reserve, status or non-status), Metis and Inuit.
Description:
The Aboriginal Patient Navigator provides support and advocacy for patients, family & community members who self-identify with Aboriginal Ancestry - First Nations (on & off reserve, status or non-status), Metis and Inuit by facilitating and coordinating access to cancer services for palliative and supportive care and addressing cultural and spiritual needs.
Services include:

Translation services in Cree, Oji-Cree and Ojibway

Support before, during or after clinical appointments

Linking patients and families to community resources

Tele-visitation services with remote family members
-
Provides information and education in a culturally sensitive manner

Acts as a liaison and advocate between the care team, patients and families

Assistance with Discharge planning
