- For persons aged 65 years and older and caregivers of older adults. - Persons aged less than 65 years old with a geriatric profile may be eligible following an evaluation of their condition. - Needs assessment and guidance toward the appropriate community resources.
Description:
Organization that informs and guides isolated seniors and caregivers towards services and resources available on the territory of MRC of Rivière-du-Nord and of Mirabel in order to break isolation, improve their quality of life and help them remain in their own homes.
An initial assessment is done by telephone to determine the general needs. Then, meetings with an intervener are organized to determine the person's needs and to set up support services to accompany them (referral to local services and resources).
