Anyone with a valid Ontario health card can be referred. Referrals can be made by the individual who needs services, a caregiver, a family member, friends, physicians or other health care professionals.

Funded by the Ministry of Health, Ontario Health at Home - North West is one of 14 Home and Community Care Support Services organizations (Community Care Access Centres) in Ontario with a focused mandate to deliver local health care services such as home and community care, access to community services and long-term care home placement.

Phone 310-2222 to be referred to a Care Coordinator in your region. Care Coordinators are regulated health professionals with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy, who work directly with patients in hospitals, doctor's offices, communities, schools and in patients' homes.
