Eligibility
H-NCSSS exists to Enhance the Independence and Improve the Quality of Life for Seniors and Adults Living with Disabilities over the age of 60 who are residents of Haldimand and Norfolk counties.
Description:
Non-profit organization providing programs and services to enhance independence and quality of life for seniors (60+) and adults with disabilities in Haldimand-Norfolk.
Meals on Wheels: Service supplying home delivery of hot and/or frozen meals to clients to promote good health, independence and social interaction.
Transportation: Transportation services provided by volunteers or staffed wheel-chair accessible vans for the purpose of attending medical appointments and other essential requirements.
Home Maintenance/Home Help: Individual home assistance with house cleaning and exterior home and yard maintenance.
Emotional Support, Volunteer Visiting: Ongoing one-to-one relationship in-home contact to facilitate social contact and monitor safety and security.
Respite, Adult Day Program: A support service which provides supervised activities for the frail elderly, and those in the early stages of dementia to achieve their maximum level of functioning in a group setting.
Hospice Palliative Care Outreach: Trained volunteers provide emotional support and companionship and for individuals living with life limiting illnesses and provide respite for caregivers.
Safety at Home: Free home assessments and recommendations to help seniors remain safely in their homes, The service includes installation of grab bars, home repair, and snow removal to prevent falls.
Emergency Response Systems: Installation and maintenance of an electronic device in the client's home for summoning emergency assistance.
