The Community Cancer Hub
Eligibility
This hub of programs and services is best suited for Ontario residents living within a 200 km radius of Ottawa and currently being treated at an Ottawa hospital. We can assist anyone living outside this area in finding another appropriate resource whenever possible.
Description:
Community Cancer Support Hub: This hub offers a single point of orientation, connecting patients and their families with numerous organizations providing non-medical community support to individuals diagnosed with cancer. This service is free and operated by the multidisciplinary team at the Community Cancer Support Hub to provide emotional, psychological, social, and symptom management support services. Many services are available, such as cancer system navigation, individual counseling, pharmacy consultations, financial assistance, movement classes, children's programs, art therapy, support groups, nutritional assistance, caregiver support, return-to-work plans, end-of-life care, and bereavement support. These various programs are designed to promote well-being and significantly enhance the quality of life for community members battling cancer.
Cancer System Navigation: The cancer system navigation program addresses the specific needs of individuals with cancer and their caregivers. One of our main priorities is to provide the community with accurate and up-to-date information on local support services for individuals with cancer. Cancer system navigators are available to listen and understand the individual needs of clients. They can help find answers to their questions or locate the right resources from diagnosis, through treatment, and beyond.
This service is available to individuals affected by cancer, including family members and/or caregivers. Services are available to people living in the Eastern Ontario region.
The navigation service is available in English and French, and free translation services are offered by Access Language Ottawa for 170 languages.
Navigators can communicate with clients via email, phone, virtually, or in person.
