Eligibility
Must be a WECHC client (primary care, diabetes wellness, respiratory wellness, active aging, any other registered program) and in need of assistance with the following: Social isolation/ exclusion (household, mental health or substance abuse) Abuse or risk of abuse Economically disadvantaged Housing (homeless or near homelessness) Mental/ physical/ emotional health challenges (short or long term/ chronic condition)
Description:
Downtown at Street Health:
711 Pelissier, Monday to Friday 7:30am-4:30pm.
Appointments are booked for any client being seen for service at this site (ie. Primary Care, Hepatitis C, Social Work etc.) Access to free NRT up to 26 weeks in 1 year span. Ongoing 1 on 1 counselling.
Sandwich Community Health Centre:

Tuesday, Thursday 7:00am – 8:00 pm
Wednesday 7:00am – 6:00pm
Appointments are booked for any client being seen for service at this site (ie. Primary Care, Dietitian, Social Work etc.) Access to free NRT up to 26 weeks in 1 year span. Ongoing 1 on 1 counselling.
