Eligibility
For children with cancer and their families.
Description:
Enables children and their families to access care and support at all stages of their illness. Community nurses help to facilitate navigation of the health care system, helping them to locate and access community and hospital resources.
Services may include:
- Discussion of diagnosis and treatment plan
- Link to the Canadian Cancer Society for assistance
- Referral to professional and volunteer community services
- Contact with child's school at the time of diagnosis and prior to the child's return
- Support for siblings, including class visits
- Coordination of care in the home if needed
- Liaison with community agency nurses and family physicians.
Assistance and support is provided through home visits, hospital visits and telephone calls.
